本課程旨在培養研究生對**高齡者整合式照護之理論、實務與管理策略**的系統性理解與應用能力。因應人口高齡化、多重慢性疾病及健康照護需求日益複雜之趨勢，課程將從高齡者健康與功能需求出發，引導學生理解整合式照護之核心理念、政策發展與服務模式，並探討其於臨床、社區及長期照護場域中的實際應用。

課程內容涵蓋**高齡者健康與功能評估、個案管理、照護協調、跨專業團隊合作、健康促進、服務流程與資源整合，以及照護品質與成效評估**等主題。透過理論講授、文獻閱讀、案例分析與問題導向討論，使學生能整合醫療、護理、復健、營養、心理、社會支持及運動健康促進等不同專業觀點，分析高齡者多面向之健康與照護需求。

完成本課程後，學生應具備**評估高齡者健康與功能需求、規劃個別化整合照護策略、進行跨專業溝通與協調，以及運用實證資料評估照護品質與成效**之能力，並能將相關知識應用於高齡健康促進、社區照護、長期照護及健康管理等實務情境。

This course aims to cultivate graduate students' systematic understanding and application ability of the theory, practice and management strategies of integrated care for the elderly. In response to the trend of an aging population, multiple chronic diseases and increasingly complex health care needs, the course will start from the health and functional needs of the elderly, guide students to understand the core concepts, policy development and service models of integrated care, and explore its practical application in clinical, community and long-term care settings.

The course content covers topics such as** health and functional assessment of the elderly, case management, care coordination, interprofessional teamwork, health promotion, service process and resource integration, and care quality and effectiveness evaluation**. Through theoretical lectures, literature reading, case analysis and problem-oriented discussions, students can integrate different professional perspectives such as medical treatment, nursing, rehabilitation, nutrition, psychology, social support and sports health promotion, and analyze the multi-faceted health and care needs of the elderly.

After completing this course, students should have the ability to** assess the health and functional needs of the elderly, plan individualized integrated care strategies, conduct inter-professional communication and coordination, and use empirical data to evaluate the quality and effectiveness of care**, and be able to apply relevant knowledge to practical situations such as health promotion of the elderly, community care, long-term care and health management.
