長照十年計畫3. 0已於今年9月上路,在2.0以社區為基礎、以人為本、連續照顧的原則下,擬定8大目標為推動主軸,持續普及照護資源、強化家庭支持及完善專業銜接,建構以居家、社區、機構及醫療體系為基礎,並整合社會福利資源之連續性照護服務模式,以達健康老化、在地安老、安寧善終之願景。
然而根據《報導者》的長年追蹤探究可知:台灣的長照制度由千禧年走來,由先導計畫打頭陣、1.0上路到2.0,25年鋪出一條讓長照走出家庭、成為公共責任的路,期盼為長照家庭卸下照顧重擔。2026年長照3.0即將全面上路,預算破千億、服務量能再擴大,這不應只是數字的疊加,更該是長照制度的新頁。
只是,長照2.0亮眼的數據掩不住現場的落差──接不住重度失能者、夜間照顧難尋、排除在給支付制度之外的住宿機構,以及隱身於體制外的外籍看護。這些缺口揭示:真正的升級,不只在統計表上,而在每一個能被看見、被支持的照顧現場。當超高齡列車全速前進,長照3.0究竟能否讓台灣抵達一個不怕老去的未來?這是一場關乎未來的革命。
長期照顧是超高齡社會的嚴峻課題,我們擬從政策演變、長照3.0、新視野、日本在宅醫療啟示、青銀共融、失智症照護、社區照顧、長照保險、經濟安全等面向進行實務探討,精選《社區發展季刊》、《公共行政學報》、《文官制度季刊》與《報導者》等論文研讀。
馮燕(2016)整理出高齡整體照顧政策架構,如上圖,主要包含高齡人口安老系統、長照服務網與長照保險等三大部份,各部份實行有其困境,有賴民間團體、自由市場與政府等通力合作。馮燕並總結道:「長期照顧服務量能提升計畫」是在轉銜過去的「長期照顧十年計畫」,並整合「長期照護服務網計畫」,根據過去政府推動長照政策與建構體系的經驗,找出突破點積極面對並解決當前長照體系發展的限制與困境,將《長期照顧服務法》各項重點工作落地紮根,秉持「公私協力」的精神,翻修原結構以積極打造全新的長照服務體系。
然而,雖然長照服務可保障失能者生存的權利及生命的尊嚴,確為實現社會正義的重要政策。但是一直箭在弦上的《長期照顧保險》仍停留在2016年的紙上談兵階段,衛福部預估我國2016 年的失能人口約為 78 萬人,約占全國總人口數 3.45%(失能率),其中老人失能人數約50萬人,約為失能人口之64%,至2060年,失能人數約為193萬人,失能率約10.54%,其中老人失能人數約 176 萬人,將占總失能人數 91%,人口高齡化的結果將使失能人數隨之增加,需要長照的人口也將隨之增加。但長照基金恐怕難以支撐日益沉重的長照成本,勢必重回長期照顧保險立法與執行之討論!
本課程希冀透過檢視長照政策的施行過程,與相關法令的修正,包括老人福利法、長期照顧服務法等,討論面臨哪些實務上的陳痾弊病,並對照其他國家的長期照顧政策之創新實例,以個案研討的方式,透過小組腦力激盪,共同為台灣的長期照顧之未來提供政策建議!

The long-term care ten-year plan 3.0 was launched in September this year. Under the 2.0 principles of community-based, people-oriented, and continuous care, 8 major goals have been formulated as the main axis to promote, continue to popularize care resources, strengthen family support, and improve professional connections, and build a continuous care service model based on home, community, institutions, and medical systems, and integrating social welfare resources to achieve the vision of healthy aging, aging in place, and a peaceful death.
However, according to the "Reporter"'s long-term follow-up research, it can be seen that Taiwan's long-term care system has been developed since the millennium, starting from the pilot project, 1.0 on the road to 2.0. In 25 years, a road has been paved to allow long-term care to go out of the family and become a public responsibility, hoping to relieve the burden of care for families. Long-term care 3.0 will be fully launched in 2026. The budget will exceed 100 billion and the service volume will be expanded. This should not just be the superposition of numbers, but also a new page for the long-term care system.
However, the dazzling statistics of long-term care 2.0 cannot hide the gaps on the ground - the inability to accommodate the severely disabled, the difficulty in finding night care, the accommodation institutions excluded from the payment system, and the foreign caregivers who are invisible outside the system. These gaps reveal: real upgrades, not just on the stat sheet, but in every care situation where they can be seen and supported. As the super-senior train moves forward at full speed, can Changzhao 3.0 allow Taiwan to reach a future where it is not afraid of aging? This is a revolution for the future.
Long-term care is a serious issue in the super-aged society. We plan to conduct practical discussions from aspects such as policy evolution, long-term care 3.0, new horizons, Japanese home-based medical inspiration, youth and silver integration, dementia care, community care, long-term care insurance, and economic security. We will select papers from "Community Development Quarterly", "Journal of Public Administration", "Civil Service Quarterly" and "The Reporter" for study.
Feng Yan (2016) sorted out the overall care policy framework for the elderly, as shown in the figure above, which mainly includes three major parts: the elderly care system, the long-term care service network, and the long-term care insurance. The implementation of each part has its own difficulties, and it depends on the cooperation of civil society, the free market, and the government. Feng Yan concluded: "The Plan to Increase the Capacity of Long-term Care Services" is a transition from the past "Ten-Year Plan for Long-term Care Services" and the integration of the "Long-term Care Service Network Plan". Based on the government's past experience in promoting long-term care policies and building a system, we will find breakthrough points to actively face and resolve the limitations and difficulties in the development of the current long-term care system, implement the key tasks of the "Long-term Care Services Act", and uphold the spirit of "public-private collaboration" to renovate the original structure to actively build a new long-term care service system.
However, although long-term care services can protect the right to survival and the dignity of life of the disabled, it is indeed an important policy to achieve social justice. However, the "long-term care insurance" that has been on the horizon is still on paper in 2016. The Ministry of Health and Welfare estimates that the number of disabled people in my country in 2016 will be about 780,000, accounting for about the total population of the country. 3.45% (disability rate), of which about 500,000 disabled elderly people account for about 64% of the disabled population. By 2060, the number of disabled people will be about 1.93 million and the disability rate will be about 10.54%. Among them, about 1.76 million disabled elderly people will account for 91% of the total disabled people. As a result of the aging of the population, the number of disabled people will increase, and the number of people who need long-term care will also increase. However, the long-term care fund may not be able to support the increasingly heavy long-term care costs, and it will inevitably return to the discussion of legislation and implementation of long-term care insurance!
This course hopes to review the implementation process of long-term care policies and the amendments to relevant laws, including the Elderly Welfare Law, the Long-term Care Services Law, etc., to discuss the practical ills faced by the elderly, and compare innovative examples of long-term care policies in other countries. Through case studies and group brainstorming, we will jointly provide policy recommendations for the future of long-term care in Taiwan!
