教育目標 Course Target

本課程旨在培養研究生對高齡者整合照護理念、模式與管理策略的系統性理解與實務應用能力。面對高齡化社會與多重慢性疾病挑戰,整合式照護成為提升健康照護品質與效率的核心策略。透過課程引導,學生將熟悉跨專業團隊合作、個案管理、服務流程設計與資源整合等關鍵議題,進一步強化其作為健康促進專業人員於臨床、社區或機構中擔任協調與管理角色的能力。 課程內涵: 整合照護基本理念與政策趨勢:介紹整合照護的核心概念、演變歷程與國內外政策發展,強化學生對制度性變革的理解。 高齡者健康與功能評估:學習常用之健康評估工具與功能評估方法,以利擬定個別化照護計畫。 個案管理與照護協調:涵蓋個案管理流程、角色與技巧,包含評估、計畫、執行、監測與評值等環節。 跨專業合作實務:探討醫療、護理、社工、營養、復健、心理等領域專業人員於照護團隊中之角色與合作模式。 品質管理與成效評估:學習照護品質指標、使用者滿意度評估與服務成效分析方法,強化學生之實證管理能力。 實務案例討論與問題解決:透過真實或模擬個案討論,訓練學生統整多元資訊、提出可行策略並進行團隊協作。This course aims to cultivate graduate students' systematic understanding and practical application ability to integrate care concepts, models and management strategies for senior citizens. Faced with the challenges of hyperagra and multiple chronic diseases, integrated care has become the core strategy to improve the quality and efficiency of healthy care. Through course guidance, students will be familiar with key topics such as cross-professional team cooperation, case management, service process design and resource integration, and further strengthen their ability to act as health promotion professionals to coordinate and manage roles in clinical, community or institution. Course content: The basic concepts and policy trends of integrated care: introduce the core concepts, evolutionary courses and domestic and foreign policy developments of integrated care, and enhance students' understanding of institutional changes. Health and function evaluation for high-age people: Learn commonly used health assessment tools and function evaluation methods to help determine an individual care plan. Case management and care coordination: covers case management processes, roles and skills, including evaluation, planning, execution, monitoring and evaluation. Cross-professional cooperation practice: Explore the role and cooperation model of professional personnel in the care team in the fields of medical care, nursing, social work, nutrition, health, psychology, etc. Quality management and effectiveness evaluation: Learning care quality indicators, user satisfaction evaluation and service effectiveness analysis methods to enhance students' practical management capabilities. Discussion on practical cases and problem-solving: train students to integrate diverse information, propose feasible strategies and conduct teamwork through real or simulated case discussions.
