教育目標 Course Target
    本課程希冀透過闡述長照2.0的施行目的、服務對象、服務內容與福利輸送與提供形式,與相關法令的探討,包括老人福利法、長期照顧服務法等共同檢視長期照顧政策,面臨哪些實務上的陳痾弊病,並對照其他國家的長期照顧政策之創新實例,以個案研討的方式,透過小組腦力激盪,共同為台灣的長期照顧之未來提供政策建議!
    雖然長照服務可保障失能者生存的權利及生命的尊嚴,確為實現社會正義的重要政策。但是一直箭在弦上的《長期照顧保險》仍停留在2016年的紙上談兵階段,衛服部預估我國2016 年的失能人口約為 78 萬人,約占全國總人口數 3.45%(失能率),其中老人失能人數約50萬人,約為失能人口之64%,至2060年,失能人數約為193萬人,失能率約10.54%,其中老人失能人數約 176 萬人,將占總失能人數 91%,人口高齡化的結果將使失能人數隨之增加,需要長照的人口也將隨之增加。但長照基金恐怕難以支撐日益沉重的長照成本,勢必重回長期照顧保險立法與執行之討論!
    聯合國於1991年提出「老化綱領」(Proclamation on Aging),提出「獨立、參與、照顧、自我實現、尊嚴」為老人應該擁有的五大核心項目,依照上述原則,世界衛生組織在2002年提出「活躍老化」(active ageing)做為高齡政策架構,主張從健康、參與以及安全三大面向,提昇高齡者的生活品質(WHO, 2002)。為了維持老人的生活品質,除了建立老年經濟安全制度外, 照顧服務體系也不可或缺,兩者相輔相成。
    衛生福利部規劃長照十年計畫2.0(簡稱長照2.0)時,推估2017 至2026 年長照需求人數之成長,2017 年長照總需求人數為 577,457 人,其中 65 歲以上老人為 416,473 人,約 72.12%,至2025年長照總需求人數為749,595人,其中,65歲以上老人為600,082人,顯現長照需求逐年增加。
    我國長期照顧服務對象是依據「經濟需求」與「照顧需求」雙重原則為服務選取標準。依據長期照顧服務法第 3條規定,長期照顧係指身心失能持續已達或預期達六個月以上者,依其個人或其照顧者之需要,所提供之生活支持、協助、社會參與、照顧及相關醫護服務。
    長期照顧十年計畫2.0(以下稱長照2.0)執行至今已八年多,推動重點包含建構居家及社區式服務為基礎之社區照顧服務體系 (長照ABC);推動長期照顧給付及支付制度,建立以個案為中心之服務模式,落實以人為中心之社區整合照顧模式,貼合使用者之長照需求;提升居家照顧服務員薪資,充實各類長照人力;逐步布建住宿式服務資源及辦理機構住民補助,提升居家式、社區式及機構住宿式服務量能,建立連續性長期照顧服務體系。
    自2016年實施「長照十年計畫2.0」,我國的長期照顧政策進入新的里程碑,然而執行面仍有諸多問題需要改革與修正!因此近日開始喊出長照3.0新願景! 
This course hopes to expound the implementation purpose, service objects, service content and welfare delivery and provision forms of Long-term Care 2.0, and discuss related laws, including the Elderly Welfare Law, Long-term Care Services Law, etc. to jointly review the long-term care policy and what practical disadvantages it faces, and compare innovative examples of long-term care policies in other countries. Through case studies and group brainstorming, we will jointly provide policy recommendations for the future of long-term care in Taiwan!
    Although long-term care services can protect the right to survival and the dignity of life of the disabled, it is indeed an important policy to achieve social justice. However, the "long-term care insurance" that has been on the horizon is still on paper in 2016. The Ministry of Health and Welfare estimates that the number of people with disabilities in my country in 2016 will be about 780,000, accounting for about 780,000 of the country's total population. 3.45% (disability rate), of which about 500,000 disabled elderly people account for about 64% of the disabled population. By 2060, the number of disabled people will be about 1.93 million and the disability rate will be about 10.54%. Among them, about 1.76 million disabled elderly people will account for 91% of the total disabled people. As a result of the aging of the population, the number of disabled people will increase, and the number of people who need long-term care will also increase. However, the long-term care fund may not be able to support the increasingly heavy long-term care costs, and it will inevitably return to the discussion of legislation and implementation of long-term care insurance!
    The United Nations proposed the "Proclamation on Aging" in 1991, proposing "independence, participation, care, self-realization, and dignity" as the five core items that the elderly should have. In accordance with the above principles, the World Health Organization proposed "active aging" as the policy framework for the elderly in 2002, advocating to improve the quality of life of the elderly from the three aspects of health, participation, and safety (WHO, 2002). In order to maintain the quality of life of the elderly, in addition to establishing an economic security system for the elderly, a care service system is also indispensable, and the two complement each other.
    When the Ministry of Health and Welfare planned the Ten-Year Long-term Care Plan 2.0 (referred to as Long-term Care 2.0), it estimated the growth in the number of people in need of long-term care from 2017 to 2026. The total number of people in need of long-term care in 2017 was 577,457, of which 416,473 were people over 65 years old, approximately. 72.12%. By 2025, the total number of people in need of long-term care will be 749,595, including 600,082 people over the age of 65, showing that the demand for long-term care is increasing year by year.
    The service selection criteria for long-term care service recipients in my country are based on the dual principles of "economic needs" and "care needs". According to Article 3 of the Long-term Care Services Act, long-term care refers to the life support, assistance, social participation, care and related medical care services provided to persons whose physical or mental disability has lasted or is expected to last for more than six months, based on the needs of the individual or his caregivers.
    The Long-term Care Ten Year Plan 2.0 (hereinafter referred to as Long-term Care 2.0) has been implemented for more than eight years. The focus of the promotion includes the construction of a community care service system based on home and community-based services. (ABC of long-term care); promote the long-term care benefit and payment system, establish a case-centered service model, implement a person-centered community integrated care model, and meet the long-term care needs of users; increase the salary of home care attendants and enrich all types of long-term care manpower; gradually deploy residential service resources and apply for institutional resident subsidies, increase the capacity of home-based, community-based and institutional residential services, and establish a continuous long-term care service system.
    Since the implementation of the "Ten-year Long-term Care Plan 2.0" in 2016, my country's long-term care policy has entered a new milestone. However, there are still many problems in implementation that need reform and correction! Therefore, we have recently begun to shout out the new vision of Changzhao 3.0!
